Book an Appointment

Please find below an outline of the steps to follow to secure your appointment with Brad Nugent at Brainstorm Assessment.

1. Carefully read the information on our services, terms and conditions and FAQs pages which contain important information about the type of booking you need to make, the relevant fees and the conditions you are agreeing to when you make your booking. It is essential that you make the correct booking. Your appointment is deemed confirmed when you book. If you have queries, please ask before booking.

2. Complete a Client Intake and Consent form. Please make sure you complete all details accurately, including the specific service type you are booking. If you have queries, please ask before completing the form. The Client Intake and Consent form can be completed securely online by clicking here.

3. Book your appointments via the online client portal. Please make sure you book the appropriate number of sessions as outlined in the FAQs. Therapy clients are advised to book a series of appointments in advance as available times fill quickly and disruption to ongoing therapy is not recommended. You can access the client portal by clicking here.

4. FOR ASSESSMENT SESSIONS ONLY: Pay the nonrefundable booking deposit to secure your appointments. We will issue you with an invoice when you complete the client intake form. The invoice must be paid within 14 days to secure your booking. Note that the deposit will be credited to your final account.

5. All done! Your scheduled appointments are now considered confirmed unless we tell you otherwise. If you need to reschedule or cancel your appointment, please note the relevant deadlines to avoid cancellation fees. Any further details will be sent to you in the week before your appointment.
